Health Examination Form and Medical Records

Before departing your home country, please:

  • Request that your home country physician complete the Health Examination Form. It is linked here and was also included with your Admissions packet. Your physician must stamp the form with the appropriate authorization. Ensure that information regarding any tuberculosis tests are complete. (TB Screen procedure and policy in English with Chinese translation here.)
  • Bring this form with you to Gannon University. This form must be on file here in order to use the health care services provided through Gannon's Student Health Center, and to arrange for the required Pennsylvania Driver License Physical Exam at no cost.
  • Remember to bring up-to-date medical records to document medical conditions requiring continuity of care and treatment. Children who will be enrolled in Erie school will be required to present a record of immunizations.
  • Undergraduate students who will live on-campus must complete the Meningitis Vaccine/Exemption Form and submit it to the Student Health Center.
  • All international students must complete a tuberculosis examination, or "screening" process upon arrival to Gannon University. The screening process may require several steps, depending upon the case. A student must complete all steps to satisfy the requirements of the screening process. Students who fail to complete all steps in a timely fashion will be suspended from the University until the screening process has been completed.

Health Insurance

All international students at Gannon University are required to carry health insurance that meets both standards set by the University and those, in the case of individuals in J-1 and J-2 status, set by the U.S. Department of State (please see your DS-2019 form for J status insurance requirements).

  • International students will be automatically enrolled in the University's group policy and billed for the cost.
  • You may waive coverage under the University group policy if you are able to present proof, in English, to the Dean of Student Development that you are enrolled for coverage under another policy with comparable terms, such as for total coverage, deductibles, provisions for repatriation, etc. Please review the Gannon University policy to compare other policies for coverage benefits and restrictions BEFORE you purchase an alternative policy. The policy for the current year is linked from the Student Health Services page.
  • Medical care in the United States can be very expensive, so it is vital for your well-being that you choose a health insurance policy with careful consideration.
